This page describes our telehealth services generally. Your signed Telehealth Informed Consent governs your care.


How We Deliver Care

All care is delivered by Theressia L. Washington, MD, board certified in Family Medicine and licensed in California, Illinois, Indiana, Montana, Nevada, and New York.

You see the same physician every visit. There is no rotating care team.

Care may be delivered by live video, telephone where clinically appropriate, secure messaging, review of uploaded photographs or documents, and remote monitoring.


Where We Practice

Services are available only to patients physically located in one of our six licensed states at the time of the visit. You must confirm your location at each visit. If you are elsewhere, the visit cannot proceed and will be rescheduled or refunded.

Tell us before your visit if you have moved or are traveling.


What Telehealth Can and Cannot Do

Telehealth works well for evaluation and management of many conditions, medication management and dose adjustment, chronic condition follow-up, laboratory ordering and interpretation, and continuity of care.

It cannot provide hands-on physical examination, procedures, imaging performed on site, or emergency care. Some conditions cannot be safely evaluated remotely, and your physician will tell you when you need to be seen in person and will refer you.


Audio-Only Visits

Telephone visits are available for some concerns where visual assessment isn't clinically necessary. Limitations:

  1. Patients located in Indiana generally require video, as Indiana does not recognize audio-only communication as telehealth for most purposes
  2. Audio-only may not be used to prescribe controlled substances
  3. Your physician may require video if audio alone is inadequate for safe care
  4. Consent to audio-only care is documented separately from video consent

Prescribing

Prescriptions are issued only after individualized evaluation and only when medically appropriate. Booking any visit does not guarantee a prescription.

We send prescriptions electronically to the pharmacy you choose. Medication cost is separate and paid to your pharmacy.

Controlled Substances

Under the DEA's fourth temporary extension of telemedicine flexibilities, Schedule II–V controlled substances may be prescribed by telehealth without a prior in-person examination through December 31, 2026, using two-way audio-video technology for a legitimate medical purpose. A permanent DEA rule is pending, and we will notify affected patients of any change.

Controlled substance prescribing requires a separate Controlled Substance Agreement, prescription monitoring program review, and possible drug screening. Your physician may decline to prescribe when it is not clinically appropriate.


Technology

You need a device with a working camera and microphone, a reliable connection, and a private location. Your physician may reschedule or change format if quality is inadequate for safe care.

We use HIPAA-compliant platforms with business associate agreements in place. We do not use consumer video tools that lack such agreements.


Privacy

Communications are encrypted and access is limited to authorized personnel. No electronic transmission can be guaranteed completely secure. You will be told if anyone else is present on our side, and you may exclude anyone unnecessary from yours.


Emergencies

Telehealth is not for emergencies.

Call 911 or go to the nearest emergency room for chest pain, difficulty breathing, severe bleeding, suspected stroke, severe allergic reaction, serious injury, or any condition needing immediate hands-on evaluation.

For thoughts of suicide or self-harm, call or text 988.

Have a plan for local emergency care. Our responsibility does not extend to emergency response.


Payment

Cash-pay. Payment due at time of service. You have the right to a Good Faith Estimate. Membership programs renew automatically and may be cancelled at any time — see Subscription & Auto-Renewal Terms.


Records

Telehealth visits are documented to the same standard as in-person care. We use more than one electronic health record platform across service lines. Your complete record includes information from every system in which you receive care, and a records request will produce from all of them.


Your Rights

You may withdraw consent to telehealth at any time without affecting future care, request in-person care instead, receive the same standard of care as in person, access your records, and receive free interpretation services, including ASL, on request.
